Question

I need a web-based tool which support for easy designing forms, role-based users for completing the forms and an embedded report and chart generator for analyzing data (if charts support drill-down its more useful). If the tool supports workflows It's good but lack of it doesn't matter.

1) What's the name for these kind of tools ?

From what you describe, it sounds like you may be looking for tools in one of a few different categories.

  1. If workflow capabilities are desireable, you'll want to look at products that fit into the "business process managment", or "BPM" category. These can be pretty complicated, however, depending on how much you need to do with them. Common examples of these are K2 BlackPoint, Skelta BPM.Net, Activiti, Bonitasoft, Sharepoint, ProcessMaker or you could even write something on top of Windows Workflow Foundation.
  2. If, however, the primary objective is just to create forms and collect data from them, you may be able to use something as simple as an HTML Web Forms package. Common examples of these are IceBrrg and WuFoo
